In addition to long-held and explicit sanitation measures this business has always adhered to, new preventative measures have been put in place to further reduce the spread of this novel coronavirus. However, these best practices still offer no guarantee regarding your potential risk of being infected.
CONSENT FOR TREATMENT: I give my consent to receive treatment from this practitioner and understand that, because services provided involve maintained touch and close physical proximity over an extended period of time, there may be an elevated risk of disease transmission, including COVID-19. By reserving this appointment, I acknowledge that I am aware of the risks involved from receiving treatment during my scheduled appointment, I voluntarily agree to assume those risks, and I release and hold harmless the practitioner/business entity from any claims related thereto.
RELEASE OF TREATMENT SESSIONS: If applicable, your continuing to book this appointment authorizes the release of all your service records on file, including the initial Health Intake form, for the purpose of processing your claims, to the following: your attorney, the healthcare provider attending to this condition, and the insurance case managers. These records will not be edited unless otherwise stated in an exclusive release of medical records signed through your attorney.
SICKNESS: Skincare and other esthetic/personal care spa services offered are not appropriate care for infectious or contagious illness. Please cancel your appointment as soon as you are aware of an infectious or contagious condition. Or if you are displaying signs or simply not feeling well; for example signs such as a cough, sore throat, headache, fever, chills, sinus or other respiratory condition. If is is within the 24-hour notice period, the cancellation fee may be waived. Amid the ongoing uncertainty of COVID-19, we have modified the cancellation policy to offer greater flexibility to all our clients. APPOINTMENT ARRIVAL: Please be sure to bring and wear your face mask, if you have forgotten yours no worries, text me at (207)595-8768 when you arrive and I will provide one for you. I will greet you at the door and guide you to a seated area to sit and take off your shoes and replace them with warm and cozy slippers. You will be asked to wash your hands and answer a few health questions, as well as get a temperature check, then get ready to relax and enjoy your service.
